A Strengths and Difficulties Questionnaire (SDQ) is designed to give us an indication of where your child is emotionally at the beginning stages of therapy. It serves as a baseline assessment, which we can return to at the conclusion of the intervention to assess the journey your child has taken with us. It also contains lots of important information that helps us design an intervention that is bespoke to you, your family, and your child’s needs.

As you will see, at the bottom of this page there is a second SDQ. This is form is for you to complete for the same reasons highlighted above.

For each item, please mark the box for "Not True", "Somewhat True" or "Certainly True". It would help us if you answered all items as best you can even if you are not absolutely certain. Please give your answers on the basis of how things have been for you over the last six months and please tick just one box for each question:
